File A Claim

The deadline to submit a Claim Form has passed. Claim Forms sent by U.S. Mail were required to be postmarked by October 4, 2025, and electronic Claim Forms were required to be submitted by 12:00 a.m. (midnight) Eastern Time on October 4, 2025. The online claims portal is now closed, and late submission of Claim Forms will not be considered for payment.

If you timely submitted a Claim Form, no further action is required at this time. If additional information is needed to process your claim, the Settlement Administrator will contact you.
